Why do I need to submit a GP Summary sheet?

We are independently regulated by NHS Health Care Improvement Scotland. They request that we attain a GP summary sheet for all patients for whom we prescribe. Thus we do need this before a Prescription is issued. This is a brief summary only detailing your diagnoses and also prescribing history, and your GP surgery should be able to print this for you.
We do not need to see or have access to your full medical records.
