ADHD Direct Responsibilities
Your prescription will only be sent once payment is received. If payment is not made during the request via JotForm, the prescription will not be written.
Your clinician will advise when to book your next medication review. Prescriptions will be stopped if reviews are not booked timely. We will remind you via email.
Report any prescription issues within 2 days of receipt, including non-receipt. Notify us in writing of any schedule changes (e.g., not needing a prescription) as soon as possible and at least 17 days before the next due date.
Keep your details updated in our system. If the address or date of birth we have on file is incorrect, inform us in writing before confirming these conditions. If a prescription is sent to the wrong address due to lack of communication, we cannot resend it ahead of schedule.
Your Responsibilities as Our Patient:
A prescription will be sent to you only after we receive both your payment and the completed form. It is your responsibility to arrange this.

Prescriptions for Controlled Drugs – What you need to know
Prescriptions expire after 28 days from their creation date. If you fail to fulfil your prescription within this time scale, no refunds will be issued, we will not be able to issue a new one unless you can provide proof of voidance.
The only proof of voidance we will accept is a legible picture of the unfilled prescription, ripped or crossed. This can be sent via email to enquiries@adhddirect.co.uk.
